Behavioural review questionnaire

Selectahead new questionnaires have been designed in line with the new Engineering and Manufacturing Apprenticeship Standards.

A combined questionnaire for the following standards:

Engineering Operative (ST0537) Level 2
Engineering Fitter (ST0432) Level 3
Engineering Technician (ST0457) Level 3

And a standalone questionnaire for the following standard:

Engineering Manufacturing Technician (ST0841) Level 4

Both questionnaires follow the same format and use internet technology to enable progress to be tracked, apprentices, assessors and employers/mentors to communicate online and generate evidence of achievement for the end point assessment (EPA). The questionnaire can be undertaken in the workplace, classroom, home, anywhere with internet access. 

Our online system enables the progress, the whole apprentice journey and the results to be shared between the Apprentice, the Assessor and the Employer/Mentor of the Apprentice. Completion and the standard being achieved is totally evidence based, signed off by all parties, ready to be downloaded in PDF format into any E-Portfolio application if required. The whole process shows total employer engagement and participation in development and achievement of the apprentice.

The candidate is reminded by email that this questionnaire is a mandatary part of their Apprenticeship Qualifications and the evidence generated will be presented at their End Point Assessment (EPA).

All parties are able to add notes/comments and sign below the Candidate’s Results Report.

All candidates are registered on the system and assigned to the Questionnaire.

• Candidate completes Questionnaire and adds comments.
• Candidates Results Report and comments automatically emailed to Assessor.
• Assessor adds comments.
• Assessor comments automatically emailed back to candidate to enable them to complete any actions that may be required.
• Assessor shares Results Report with Employer/Mentor.
• Employer/Mentor add their comments.
